Teenagers Abroad Forms

All forms needed to enroll in a Teenagers Abroad program can be found on this page directly below.

Teenagers Abroad now offers you the option to either download, print and fax/mail us the forms or simply complete and submit them online.

Enrollment Form:
In order to enroll in a program, all Teenagers Abroad participants must either complete & sign this form, print proof of deposit and/or full payment, and fax these documents to us or complete and submit all forementioned documents online.

Teen Waiver Form:
Parents/guardians must complete this form if the participant enrolling in a Teenagers Abroad program is less than 18 years old. Please complete & submit this form online or sign and fax it to us along with the above enrollment form (mandatory) and credit card form (if applicable).

Permission to Leave Campus Form:
We do not allow any students younger than 16 years old to leave the school or campus by themselves without written or confirmed permission of their parents/guardians. Complete the Permission to Leave Campus form if you wish to give your child permission to leave the school campus at certain times without adult supervision.

Credit Card Form:
For those wishing to pay their program’s deposit or pay for their program in full with a credit card, print, complete & sign this form and fax it to us with the enrollment form found above.

Flight Details Form:
In order to ensure that we have a Teenagers Abroad staff member at the airport or train station to meet your teen upon arrival at their final destination, we must be provided with his/her flight/travel information at least 2 weeks before his/her program’s start date. Inform us of all relevant travel information by filling out this simple form online.

Academic Credit Information Request Form:
If you are interested in taking one of our Teenagers Abroad programs and applying for undergraduate academic credit, please complete and submit this form.

Authorization for Enrollment of Concurrent High School Student:
After completing the Information Request Form for academic credit (directly above), high school students must also download, complete and fax this form.
