High School Student Authorization Form

If you are interested in participating in one of our Teenagers Abroad summer camp program in order to earn undergraduate academic credit from Brookhaven College, please complete & submit the Academic Credit Information Request Form online.

Once you have submitted the above form, please Download, Complete and Fax the following form:
>> Authorization for Enrollment of High School Concurrent Student
